Service Description

Your first visit is the most important. It is a very thorough visit. It is probably more thorough than any other medical visit you have ever been to as it will last 3 hours. Unlike most other visits, when you come in to see us the first time, we want to find out as MUCH about YOU and what is BOTHERING you as we can. That includes discussing in details much about your life, habits and drawbacks. The more we know, the better your end results will be. There will be a 4-page health questionnaire to fill out as well as other forms. This form is available for download, however we request that you fill it out and email it back to us so that we may print it out on our color coded paper. You are also, however, welcome to let us know you would like to stop by and pick up the forms to fill out at home. Please understand your appointment could still take 2-3 hours.
